Examples of errors include:

  • Prescribing the wrong medication or the wrong strength

  • Dispensing the wrong dosage
  • Labeling errors (wrong drug on label, wrong instructions)
  • Failing to note adverse drug interactions of multiple drugs prescribed
  • Improperly administering the drug
